چکیده

Introduction Determining the level of atrioventricular [AV] block noninvasively can be challenging (ie, AV nodal versus His-Purkinje system). Presyncope or syncope in the setting of AV block is often assumed to be due to progression of the conduction disease. We describe a woman with second-degree AV block with narrow complex QRS with unclear level of AV block. She developed pause-dependent torsade de pointes in the setting of the AV block.

Prevention of torsade de pointes in the congenital long QT syndrome: use of a pause prevention pacing algorithm.

Torsade de pointes in the congenital long QT syndrome (LQTS) is often pause dependent. Thus, the main goal of pacemaker treatment in the LQTS may be the prevention of pauses that facilitate the onset of torsade de pointes. A pause prevention pacing algorithm (rate smoothing) was used for arrhythmia prevention in a 14 year old girl with congenital LQTS. By temporarily increasing the pacing rate ...

Arrhythmias in the congenital long QT syndrome: how often is torsade de pointes pause dependent?

OBJECTIVE To determine the frequency and predictors of pause dependent torsade de pointes among patients with the congenital long QT syndrome and spontaneous ventricular tachyarrhythmias. DESIGN The literature on the "congenital long QT" was reviewed. Articles with illustrations demonstrating the onset of spontaneous polymorphic ventricular arrhythmias in the absence of arrhythmogenic drugs w...

[Pause-dependent Torsade de Pointes following acute myocardial infarction].

The authors report an early occurrence (within 48 hours) of the newly described form of pause-dependent torsade de pointes following acute myocardial infarction. QT prolongtion after myocardial infarction a common ECG finding. Postextrasystolic QT prolongation rare but one of the warning signs for torsade de pointes ventricular tachycardia following myocardial infarction. Careful monitoring is ...

A practical approach to torsade de pointes.

The term torsade de pointes refers to polymorphic ventricular tachycardia that occurs in the setting of an abnormally long QT interval. While the most common cause is treatment with QT prolonging drugs, torsade de pointes also occurs in the congenital long QT syndromes and in the setting of acquired heart block or severe electrolyte disturbance, notably hypokalemia. Among QT prolonging drugs th...

Amiodarone-Induced Third Degree Atrioventricular Block and Extreme QT Prolongation Generating Torsade Des Pointes in Paroxysmal Atrial Fibrillation.

Amiodarone is still the most potent antiarrhythmic drug in the prevention of life threatening ventricular arrhythmias and demonstrates a very low incidence of torsade de pointes. An unusual case of an 81-year-old woman who developed serious abnormalities of the conduction system of the heart and torsade des pointes during intravenous infusion of amiodarone for the treatment of paroxysmal atrial...
